Dear Patients,

I am writing to inform you that Morrison Clinic Psychiatry will be closing permanently, effective July 29, 2026.

It has been a privilege to be part of your care, and I am deeply grateful for the trust you have placed in me. This decision was not made lightly, and I understand that transitions in care can be challenging.

Continuation of Care

To support continuity of care, medical appointments for medication refills and transition of care will remain available through July 29, 2026. These appointments will be offered on a first-come, first-served basis. I encourage you to schedule as soon as possible and to establish care with another qualified mental health provider. You may consider contacting your insurance provider for a list of in-network clinicians or using reputable directories such as Psychology Today or your local medical society.

Medication Refills

Prescription refills will only be provided in the context of a scheduled medical appointment and will not be available after July 29, 2026.

Medical Records

You have the right to request a copy of your medical records or have them sent to another provider.

Your PatientFusion.com portal will remain accessible for 30 days following your last appointment or 30 days from the date of your discharge letter. You are encouraged to log in and download a copy of your records as soon as possible.

After the portal closes, medical records are maintained by:

To request records or obtain current custodian information, please submit a written request by fax to:
(847) 787-1437

Your request should include:

• Patient name, date of birth, and phone number
• Description of request (records or custodian information)
• Where records should be sent (if applicable)
• Signature and date

If custodianship has changed, updated custodian information will be provided in response to your request.

A valid written authorization is required before records can be released. If your request does not meet HIPAA authorization requirements, a Release of Information form will be faxed to you for completion.

If you are experiencing a mental health emergency, please call 911 or go to your nearest emergency room.
