Morrison Clinic New Patient Intake Procedures


Morrison Clinic is currently accepting new patients aged 18-64 for virtual telepsychiatry medical care.

Please note that we do not offer face-to-face medical care at this time and only offer telehealth. In line with this, our practice has fully embraced online systems for new patient screening and scheduling, and we do not provide these services by telephone. We have found that potential patients are often not available by phone during our business hours. To enhance access to care beyond typical business hours, Morrison Clinic has invested in an online system for new patient intake and scheduling which can be accessed by future patients every day of the week, at any time, day or night.


The Morrison Clinic online New Patient Intake includes 4 modules that the patient can submit at their own pace.  Each module is intended to take ~10-20 minutes to complete.

MODULE 1: NEW PATIENT SCREENING

Our screening process is designed to ensure that our clinic is the right fit for your medical needs, helping to prevent patients from incurring copays and appointment fees only to be referred elsewhere. This process helps us match you with the appropriate care and ensures that our clinician and practice can effectively meet your healthcare requirements.  

MODULE 2: ADMINISTRATIVE INFORMATION

This module includes consent forms, demographic information, and an estimate of out-of-pocket costs for self-pay or insurance.

MODULE 3: MEDICAL HISTORY

Here, you will provide your past psychiatric and medical history.

MODULE 4: PSYCHOLOGICAL TESTING

This module involves psychological testing to establish a baseline for your depression, anxiety, sleep, and cognitive symptoms.

After you submit the fourth module, our staff will take about 1-2 business days to process your new patient intake and schedule a 60-minute virtual medical appointment with our provider.
